Holland Positions: 

Physical Therapy Assistant – Full Time Position
LifeCircles-PACE of Holland, MI is currently seeking a full time physical therapy assistant to join our innovative and interdisciplinary program serving seniors residing in community settings.In this position the PTA assesses and executes the treatment plan of the PT based on the participant’s mobility level and functional ability; provides therapies to maximize independence and helps maintain safety in their living environment. They are responsible for assisting in the overall needs of the therapy department which includes providing therapeutic activities/exercise, transfer/gait training, monitoring and maintaining DME,  and the planning/development of group and one to one treatment sessions.
The PTA must have:
• experience worked with the frail/elderly,
• a current Michigan license as a Physical Therapy Assistant
• previous home care experience is preferred.

Certified Nursing Assistant 

“LifeCircles, A Porter Hills PACE Partnership, is in need of a full time Certified Nursing Assistant to join our growing team!  Be a part of our innovative geriatric health care program in Holland, Michigan. This position is Monday-Friday with no nights or weekend hours, 40 hours/week is guaranteed, with benefits!
The successful candidate will have a passion for providing care for the elderly.  LifeCircles CNA’s work with participants both in the center and on the bus and will be responsible for providing support with ADL and IADL needs while at LifeCircles. Job duties will include taking vital signs, reporting significant changes in condition, assisting with Recreation, Physical, and Occupation Therapy as needed, assisting in the safe transport of participants to and from the day center, and performing routine tasks and duties as assigned.

Candidates must be currently certified as a nurse aide in the State of Michigan, and must have at least one year of experience in working with the frail elderly. CPR certification and updated immunization status is required or will be provided.”

Muskegon Positions: 

 

Certified Nurse’s Aide – Home Care

LifeCircles, A Porter Hills PACE Partnership, is hiring full-time nurse aides to join our innovative, allinclusive geriatric health care program in Muskegon, Michigan. The full- time positions will include day and evening hours. The home care nurse aide plays a critical role in assisting participants with personal cares, maintaining health and function, and significantly contributing to their desire to remain at home. A successful candidate will arrive to work with enthusiasm, greet our participants with grace, connect and communicate with empathy, and be a supportive member of the team. Candidates must have a current and valid certification to work as a nurse aide in the State of Michigan and must have at least one year of experience in working with the frail elderly. Home care experience is preferred. Candidates must possess an unrestricted driver’s license and have a reliable vehicle with valid insurance coverage. CPR certification and updated immunization status is required or will be provided.

Registered Dietitian

LifeCircles, A Porter Hills PACE Partnership, is hiring a full-time community focused Registered Dietician to join our innovative, all-inclusive geriatric health care program in Muskegon, Michigan. Life Circles is a community based program that supports participants and their care partners by providing compassionate, comprehensive, team based care focused on health, function, and the participant’s desire to remain living at home. The Registered Dietician provides nutritional services to participants to include: completing assessments/reassessments, developing a nutritional plan of care in line with participant centered goals of care, facilitating participant and care partner education with an emphasis on eating habits/preferences and meal planning, and operating as part of an interdisciplinary model of care that fosters shared decision-making responsibilities. Candidates must possess a Bachelor’s Degree from a Commission an Dietetic Registration accredited institution with a major in food and nutrition or dietetics and have at least one-year experience working with the frail elderly in a community based setting or long-term care setting. Board Certification in gerontological nutrition, renal nutrition, enteral/parenteral nutrition, or diabetes education is preferred but not required.

Recreational Therapy Assistant – Part Time

Life Circles is a community based program that supports vulnerable adults and their care partners by providing compassionate, comprehensive, team based care that honors their health care preferences and their desire to remain living in the community

The Recreation Therapy assistant working in conjunction with the Activity Coordinator will be responsible for the planning and the implementation of recreational programs based on participant interests and needs to help support them to maintain an optimal level of physical, social and emotional functioning to be able to remain in their home.

The RTA encourages each participant’s autonomy and dignity by engaging them in meaningful activity in a group or with independent pursuits.

The candidate must have at least one year of experience working with the frail elderly and prefer experience in planning and leading activities. Must have a high school diploma or equivalent.
