Certified Nursing Assistant 

“LifeCircles, A Porter Hills PACE Partnership, is in need of a full time Certified Nursing Assistant to join our growing team!  Be a part of our innovative geriatric health care program in Holland, Michigan. This position is Monday-Friday with no nights or weekend hours, 40 hours/week is guaranteed, with benefits!
The successful candidate will have a passion for providing care for the elderly.  LifeCircles CNA’s work with participants both in the center and on the bus and will be responsible for providing support with ADL and IADL needs while at LifeCircles. Job duties will include taking vital signs, reporting significant changes in condition, assisting with Recreation, Physical, and Occupation Therapy as needed, assisting in the safe transport of participants to and from the day center, and performing routine tasks and duties as assigned.

Candidates must be currently certified as a nurse aide in the State of Michigan, and must have at least one year of experience in working with the frail elderly. CPR certification and updated immunization status is required or will be provided.”

Muskegon Positions: 

Quality Improvement Coordinator

Life Circles is a community based program that supports vulnerable adults and their care partners by providing compassionate, comprehensive, team based care that honors their health care preferences and their desire to remain living in the community. We are seeking a full time Quality Improvement Coordinator to join our innovative Quality team in Muskegon, MI.  Our goal in Quality is to have a data driven quality improvement approach to support exceptional participant care.  The main focus of this position will involve assisting the Quality Manager in auditing, contracts management, revising policies, tracking ongoing staff education and assisting with audit/survey preparation reports, as well as other administrative assistant tasks. This role requires a high attention to detail and the ability to follow directions. Aspects of Quality Coordinator position may include:

  • Coordinating annual review of and reformatting of all policies/procedures
  • Assists with preparation for certification surveys, gathering information and organizing data and documents
  • Coordinates employee training program, manages on-line training schedule
  • Assists with new employee orientation
  • Track and report organization compliance with Infection Control policy and protocols
  • Assists with maintaining contracted provider files and the network database and assists with ongoing monitoring of the contracted providers
  • Routine auditing of the electronic health record
  • Responsible for taking minutes during monthly QAPI meetings
  • Assists with the preparation and implementation of the organization’s Emergency Preparedness Plan

Qualifications and Experience:

Required:

  • Excellent MS word and MS Excel skills
  • MS Outlook and Web meeting knowledge
  • Attention to detail
  • Strong communication skills
  • Highly organized
  • Strong spelling and grammar skills
  • Two years’ experience in filing both paper and electronic documents
  • Associates Degree, or college credit equivalent

Preferred:

  • Previous experience in an office setting
  • Good Clinical Practice or related regulatory training
  • Experience with clinical or long term care audits
  • Experience with Emergency Preparedness Planning

Beginning Hourly Compensation: $17.50/hour; hourly rate may be negotiable based on relevant experience.

Social Work Supervisor

Role and Responsibilities

Life Circles is a community-based program that supports vulnerable adults and their care partners by providing compassionate, comprehensive, team-based care that honors their health care preferences and their desire to remain living in the community. We are seeking a full time Social Work Supervisor to join our innovative Interdisciplinary Team in Muskegon, MI.  The main focus of this position is to supervise a team of Social Workers and to plan, coordinate, monitor and ensure the provision of high quality Social Work services that are effectively integrated with an evolving behavioral health management system and interdisciplinary team-based model of care.  This includes providing direct and indirect social work services to participants and care partners and simultaneously managing evolving social work case management functions that are fully integrated with LifeCircles behavioral health system.  The position is independent in the application of advanced social work knowledge and skills and able to manage complex clinical situations for which he/she is responsible in an efficient manner and in accordance with established agency defined procedures, social work best practices, and LifeCircles mission and vision.  Additionally, this position may supervise both licensed and non-licensed personnel within the Social Work department as well as coordinate interns and volunteers.

Qualifications and Education Requirements

Must possess a fully Licensed Master’s degree in social work (LMSW) from a program accredited by the Council on Social Work Education (CSWE);

  • Have a minimum of one year of experience working with the frail elderly, or long-term care population.

Preferred Skills

  • Medical Social Work experience, supervisory experience and participation in an interdisciplinary team-based model of care.

Certified Nurse’s Aide – Home Care – Full Time

CENA – Home Care
LifeCircles, A Porter Hills PACE Partnership, is hiring full-time nurse aides to join our innovative, all-inclusive geriatric health care program in Muskegon, Michigan.
The full- time positions include a 3-day, 36 hour week, working 8:00 am to 8:00 pm and a second shift position, 2:00 pm to 10:00 pm, Monday through Friday.
The home care nurse aide plays a critical role in assisting participants with personal cares, maintaining health and function, and significantly contributing to their desire to remain at home. A successful candidate will arrive to work with enthusiasm, greet our participants with grace, connect and communicate with empathy, and be a supportive member of the team.
Candidates must have a current and valid certification to work as a nurse aide in the State of Michigan and must have at least one year of experience in working with the frail elderly. Home care experience is preferred. Candidates must possess an unrestricted driver’s license and have a reliable vehicle with valid insurance coverage. CPR certification and updated immunization status is required or will be provided.

Recreational Therapy Assistant – Part Time

The Recreation Therapy assistant working in conjunction with the Activity Coordinator will be responsible for the planning and the implementation of recreational programs based on participant interests and needs to help support them to maintain an optimal level of physical, social and emotional functioning to be able to remain in their home.

The RTA encourages each participant’s autonomy and dignity by engaging them in meaningful activity in a group or with independent pursuits.

This position is part-time; 4 days a week, 9:30 am – 3:30 pm with time occasionally flexing to meet departmental needs.

The candidate must have at least one year of experience working with the frail elderly and prefer experience in planning and leading activities. Must have a high school diploma or equivalent.
