Behavioral Health Specialist

The PACE behavioral health specialist (BHS) provides a range of behavioral health services including assessment, psychotherapy, health behavior interventions, interdisciplinary team consultation, and staff training. The BHS works closely with an interdisciplinary team including doctors, nurses, physical/occupational/speech therapists, case managers, and other day center staff to creatively optimize functioning of participants, support coping through chronic disease, and helping people thrive in the community.

Responsibilities include but not limited to:

  • Interdisciplinary team consultation and staff training
  • Biopsychosocial and differential diagnostic assessment
  • Mental health treatment planning to optimize wellness and preserve placement in the community
  • Direct (individual and group) behavioral health services across a variety of settings including the day center, participant’s homes, acute and long-term care
  • Program development and quality improvement initiatives related to person-centered care and psychosocial wellness of participants, care partners, and staff.

Qualifications:

  • Masters Degree in Counseling Psychology, Clinical Psychology, Applied Psychology, Social Work, Marriage & Family Therapy, Mental Health, or equivalent.Current license to practice behavioral health services in Michigan.Strong critical thinking and creativity skills.
  • Experience or training in:Providing behavioral health services such as assessment, psychotherapy, counseling, and behavioral consultation
  • Working with older adults, people with serious mental illness, and/or people with disabilities
  • Working in medical settings and with integrated health teams Preferred Skills:
  • Knowledge of evidence-based practice and comfort with integrated approaches to care
  • Experience with substance use treatment and dialectical behavior therapy a plus

Behavioral Health Liaison

Life Circles is a community based long term care program that supports vulnerable older adults and their care partners by providing compassionate, comprehensive, team based care that honors their health care preferences and their desire to remain living in the community. One element that can threaten community living and present difficulties across care settings are challenging behavioral issues. These issues require increased complexity of care plan design and delivery, as well as growing knowledge and expertise of all staff and care partners. LifeCircles is seeking a Behavioral Health Liaison (BHL) to providing additional consultation to team members, promote close coordination with all gatekeepers, and support excellent implementation of behavior plans.

Aspects of BHL position may include:

  • Assist with the development and monitoring of both person-centered and system-wide behavioral programs that promote psychosocial wellness
  • Train/coach team members and care partners in implementation of complex behavioral care plans
  • consult about strategies that promote interdisciplinary teamwork and advance team wellness and functioning

Qualifications and Experience:

  • Bachelor’s degree plus 5+ years related experience or Masters Degree in relevant field. This includes Social Work, Counseling Psychology, Clinical Psychology, Behavior Analysis, Applied Psychology, Marriage & Family Therapy, Mental Health, or equivalent

Required

  • A minimum of one year experience working with frail older adults or the long term care population
  • Training/experience in supporting teamwork, staff engagement, and positive organizational culture changes
  • Experience in providing behavioral consultation and staff training
  • Experience working in medical settings and with integrated health teams
  • Experience working with people with serious mental illness, and/or intellectual or developmental disabilities
  • Excellent interpersonal and communication skills

Preferred

Holland Positions:


Occupational Therapy Assistant – Part Time

LifeCircles, a Porter Hills PACE Partnership, is in need of a part-time OTA to join our innovative geriatric health care program in Holland, MI.

LifeCircles provides all-inclusive care to frail seniors in the community who desire to remain in their home but need additional support to achieve that goal. The PACE OTA provides a range of services and serves as an essential role in coordinating participant’s complex needs. The PACE OTA works closely with an interdisciplinary team including doctors, social workers, therapies and day center staff to assist the participant in achieving their goals of care.

Role and Responsibilities

Primary role responsibilities would include:

  • Assisting in overall needs of therapy department, including therapeutic activities/exercise, activity modification and engagement, monitoring and maintaining DME, planning/development of group and one on one treatment sessions.
  • Collaboration with interdisciplinary team to problem-solve care needs
  • Communication between participants and care partners as appropriate

Qualifications and Education Requirements

  • Current certification as Occupational Therapy Assistant (COTA)
  • Current licensure as Occupational Therapy Assistant in the State of Michigan.
    1 year experience with the frail elderly
  • Preferred outpatient/clinic experience as OTA

RN Case Manager

Life Circles Holland PACE is seeking a full-time RN Case Manager to work Monday through Friday with on-call rotation requirements. Responsibilities include but not limited to:

  • Case management to assess, plan, implement, and continually evaluate plan of care for patients age 55 years and older.
  • Regular collaboration with interdisciplinary team and other community organizations to ensure patients remain safe/successful at home and prevent unnecessary hospitalizations.
  • Primarily clinic-based care (8:00 am to 4:30 pm) with opportunities for community and home visits.
  • Rotating on-call with additional compensation.

Minimum qualifications for the position:

Must have high ethical standards.

Current RN license in Michigan required, driver’s license, and auto insurance. Must provide proof to office.

Must possess, as a minimum, an RN Nursing Degree from an accredited college or university or be a graduate of an approved RN program.

Clinic Nurse/LPN

ROLE AND RESPONSIBILITIES

Primary role responsibilities would include:

  • Provide nursing care to patients aged 55 and older in an adult day center and outpatient clinic setting (8:00-4:30)
  • Regular collaboration with CNAs, RNs, MAs, medical providers, and other members of the interdisciplinary team to ensure patients nursing care plan needs are met.
  • Identify patient needs in person and over the phone, provide nursing care, administer medications, collect lab samples, and complete treatments under the direction of an RN or medical provider.
  • Provide education to patients and families

QUALIFICATIONS AND EDUCATION REQUIREMENTS

LPN Nursing Diploma or Degree from accredited School of Nursing or University

Medical Records Technician

Maintain all medical records to ensure compliance with HIPPA standards and regulatory requirements.

  • Collect, Monitor, Scan and Index all paper records.
  • Routine auditing of medical records, both electronic and paper.

  • Maintains and processes all releases of information (ROI)

Recreational Therapist

LifeCircles-PACE is in need of a full-time Recreational Therapist to join our innovative geriatric health care program in Holland, MI. LifeCircles provides all-inclusive care to frail seniors in the community who desire to remain in their home but need additional support to achieve that goal. The PACE Recreational Therapist works closely with an interdisciplinary team including doctors, social workers, therapies and day center staff to assist the participant in achieving their goals of care.

Our Adult Day Center is open Monday-Friday from 8:00am-4:30pm and closed on major holidays and participants are brought to the Day Center with LifeCircles’ transportation. While at the Adult Day Center, participants also have access to our medical clinic, rehab services and social work.

Role and Responsibilities

Primary role responsibilities would include:

  1. Conducting Annual Assessments
  2. Developing, planning, and leading activity programs in the Adult Day Center
  3. Work with participants to develop individual care plans to meet their recreation and leisure goals

Enrollment Coordinator

Life Circles is a community-based program that supports vulnerable adults and their care partners by providing compassionate, comprehensive, team-based care that honors their health care preferences and their desire to remain living in the community. We are seeking a full time Enrollment Coordinator to join our Marketing and Enrollment team in Holland, MI. The primary purpose of this position is to coordinate enrollments for potential participants in a manner that aligns with LifeCircles growth goals. This position also maintains community referral relationships and supports the overall intake and community outreach activities of LifeCircles.

Aspects of Enrollment Coordinator position may include:

  • Accepting and logging referrals for potential new enrollees
  • Providing initial evaluations and home visits to screen for eligibility into the PACE program and providing eligibility information to both the participant and care partners
  • Assisting potential enrollees and their care partners in applying for Medicaid, if eligible
  • Communicating with the Interdisciplinary Team (IDT) about timelines and new enrollee deadlines
  • Ensuring necessary paperwork related to participant enrollment is received by all parties before appointed cut-off date
  • Assisting with community engagement and marketing activities as directed by the Marketing and Enrollment Manager or other marketing staff

Qualifications and Experience:

Required:

  • Bachelor’s Degree (health-related field preferred)
  • Minimum 2 years of experience in outreach/intake or care management in a health care setting
  • Minimum 1 year of experience working with the frail, elderly, or Long-Term Care population
  • Ability to establish relationships with people from various backgrounds

Muskegon Positions: 

Certified Nurse’s Aide – Home Care – Full Time
LifeCircles, A Porter Hills PACE Partnership, is hiring full-time nurse aides to join our innovative, all-inclusive geriatric health care program in Muskegon, Michigan.

The home are nurse aide plays a critical role in assisting participants with personal cares, maintaining health and function, and significantly contributing to their desire to remain at home.  A successful candidate will arrive to work with enthusiasm, greet our participants with grace, connect and communicate with empathy, and be a supportive member of the team.
The full- time positions include 8 am -4:30 pm Monday through Friday and a second shift position, 2:00 pm to 10:00 pm, Monday through Friday.
Candidates must have a current and valid certification to work as a nurse aide in the State of Michigan and must have at least one year of experience in working with the frail elderly. Home care experience is preferred. Candidates must possess an unrestricted driver’s license and have a reliable vehicle with valid insurance coverage. CPR certification and updated immunization status is required or will be provided.

Facilities Maintenance Assistant

LifeCircles is in need of a full-time facilities maintenance assistant to join our health care program in Muskegon, MI.

The successful candidate’s responsibilities will include assisting the LifeCircles facilities manager and perform routine maintenance and repair of the facilities and its equipment to include minor plumbing, electrical, carpentry, mechanical, etc.  Provide support to the LifeCircles staff and perform many routine tasks and duties as assigned.

Must possess, as a minimum, a high school education and have at least one year of experience working with seniors or the frail and elderly population.
