Holland Positions:

LifeCircles – PACE of Holland looking for a Registered Nurse (RN) to join our innovative Geriatric Health Care Program in Holland, MI.

LifeCircles provides all-inclusive care to frail seniors in the community who desire to remain in their home but need additional support to achieve that goal. Geriatric appropriate clinical services are provided both in the onsite clinic and in patient’s homes. This position would work closely with an interdisciplinary team including clinical staff, behavioral health, therapies and day center staff to deliver needed services with a strong emphasis on patient guided care.
Role and Responsibilities

Primary role responsibilities would include:

  1. Ensuring clinical needs are addressed both in the home and in the clinic
  2. Facilitating transitions of care surrounding hospitalizations
  3. Participant Assessments in their homes, as well as a variety of care settings
  4. Staff, Caregiver, and Participant education regarding disease management and health outcomes
  5. Coordinating care across various settings and proactively addressing health concerns

Qualifications and Education Requirements

Nursing Degree from an accredited School of Nursing BSN preferred
1 year experience with the frail elderly

Marketing and Enrollment Manager

LifeCircles, a Porter Hills PACE Partnership, is in search of a Marketing and Enrollment Manager to join our innovative health care program in West Michigan.

LifeCircles is a fast-growing Program of All-Inclusive Care for the Elderly (PACE) provider serving Allegan, Ottawa, and Muskegon counties. LifeCircles supports vulnerable adults and their caregivers by providing compassionate, comprehensive, coordinated care that honors their health care preferences and the desire to live at home. The primary purpose of this position is to ensure LifeCircles achieves enrollment goals through targeted measurable marketing efforts, by overseeing marketing and enrollment processes, and supervising enrollment staff.

 

Role and Responsibilities

Primary role responsibilities would include:

1.       Leads and operationalizes the strategic objectives for LifeCircles marketing and enrollment functions, maximizing market penetration.

2.       Develops annual marketing plan and all appropriate marketing materials, including newsletters, press releases, advertising, and social media.

3.       Evaluates the effectiveness of marketing efforts, adjusting the plan as indicated.

4.       Provides individual and group marketing presentations to a wide variety of audiences, participates in events, open houses, and executes additional marketing activities.

5.       Provides supervision to enrollment staff.

6.       Develops and maintains positive collaborative relationships with referral sources, Medicaid offices, and LifeCircles staff members.

 

Qualifications and Education Requirements

1.       Bachelor’s Degree

2.       Minimum of 2 years’ experience in a sales or marketing role for a health care provider or human services organization

Certified Nurse’s Aide – Home Care – Full Time
LifeCircles, A Porter Hills PACE Partnership, is hiring full-time nurse aides to join our innovative, all-inclusive geriatric health care program in Muskegon, Michigan.

The home are nurse aide plays a critical role in assisting participants with personal cares, maintaining health and function, and significantly contributing to their desire to remain at home.  A successful candidate will arrive to work with enthusiasm, greet our participants with grace, connect and communicate with empathy, and be a supportive member of the team.
The full- time positions include 8 am -4:30 pm Monday through Friday and a second shift position, 2:00 pm to 10:00 pm, Monday through Friday.
Candidates must have a current and valid certification to work as a nurse aide in the State of Michigan and must have at least one year of experience in working with the frail elderly. Home care experience is preferred. Candidates must possess an unrestricted driver’s license and have a reliable vehicle with valid insurance coverage. CPR certification and updated immunization status is required or will be provided.

Recreational Therapy Assistant – Part Time

Life Circles is a community based program that supports vulnerable adults and their care partners by providing compassionate, comprehensive, team based care that honors their health care preferences and their desire to remain living in the community

The Recreation Therapy assistant working in conjunction with the Activity Coordinator will be responsible for the planning and the implementation of recreational programs based on participant interests and needs to help support them to maintain an optimal level of physical, social and emotional functioning to be able to remain in their home.

The RTA encourages each participant’s autonomy and dignity by engaging them in meaningful activity in a group or with independent pursuits.

This position is part-time; 4 days a week, 9:30 am – 3:30 pm with time occasionally flexing to meet departmental needs.

The candidate must have at least one year of experience working with the frail elderly and prefer experience in planning and leading activities. Must have a high school diploma or equivalent.

Certified Nurse’s Aide

LifeCircles, A Porter Hills PACE Partnership, is in need of a full time Certified Nursing Assistant to join our growing team!  Be a part of our innovative geriatric health care program in Muskegon, Michigan. This position is Monday-Friday with no nights or weekend hours, 40 hours/week is guaranteed, with benefits!
The successful candidate will have a passion for providing care for the elderly.  LifeCircles CNA’s work with participants both in the center and on the bus and will be responsible for providing support with ADL and IADL needs while at LifeCircles. Job duties will include taking vital signs, reporting significant changes in condition, assisting with Recreation, Physical, and Occupation Therapy as needed, assisting in the safe transport of participants to and from the day center, and performing routine tasks and duties as assigned.

Candidates must be currently certified as a nurse aide in the State of Michigan, and must have at least one year of experience in working with the frail elderly. CPR certification and updated immunization status is required or will be provided.

MSW – Social Worker

LifeCircles, a Porter Hills PACE Partnership, is in need of a full-time social worker to join our innovative geriatric health care program in Muskegon, MI.

LifeCircles provides all-inclusive care to frail seniors in the community who desire to remain in their home but need additional support to achieve that goal. The PACE social worker provides a range of services and serves as an essential role in coordinating participant’s complex needs. The PACE social worker works closely with an interdisciplinary team including doctors, nurses, therapies and day center staff to assist the participant in achieving their goals of care.

Role and Responsibilities

Primary role responsibilities would include:

  1. Advance Care Planning
  2. Facilitating transitions of care
  3. Participant Assessments in homes as well as a variety of care settings
  4. Mental Health Case Management
  5. Staff, Caregiver, and Participant education regarding end of life, disease management and caregiver stress

Behavioral Health Specialist

The Behavioral Health Specialist (BHS) provides a range of behavioral health services including assessment, psychotherapy, health behavior interventions, interdisciplinary team consultation, and staff training.

Responsibilities include but not limited to:

  • Interdisciplinary team consultation and staff training
  • Biopsychosocial and differential diagnostic assessment
  • Mental health treatment planning to optimize wellness and preserve placement in the community
  • Direct (individual and group) behavioral health services across a variety of settings including the day center, participant’s homes, acute and long-term care
  • Program development and quality improvement initiatives related to person-centered care and psychosocial wellness of participants, care partners, and staff.

Qualifications

  • Masters Degree in Counseling Psychology, Clinical Psychology, Applied Psychology, Social Work, Marriage & Family Therapy, Mental Health, or equivalent
  • Current license to practice behavioral health services in Michigan
  • Strong critical thinking and creativity skills
  • Experience or training in:
    • providing behavioral health services such as assessment, psychotherapy, counseling, and behavioral consultation
    • working with older adults, people with serious mental illness, and/or people with disabilities
    • working in medical settings and with integrated health teams

  Preferred Skills

 Knowledge of evidence-based practice and comfort with integrated approaches to care

  • Experience with substance use treatment and dialectical behavior therapy a plus
