Enrollment into LifeCircles PACE® is as easy as starting with a phone call to our Intake Department at 231-733-8655 or 616-582-3100. We will ask questions about the challenges being experienced to help determine if our program is the right choice. If LifeCircles is not the right choice, we’ll happily give suggestions of other area resources. If it’s determined that LifeCircles is the right program, we’ll start the Intake Process. LifeCircles is not a ‘sign up today, start tomorrow’ option of care.
All LifeCircles participants meet a level of care that qualifies them to live in a long term care nursing home; however, with community and PACE supports, they are able to live safely in the community.
People wishing to enroll in LifeCircles PACE must meet the following criteria:
- Be age 55 or older,
- Live in anywhere in Muskegon County and portions of Ottawa and Allegan Counties
- Meet the Nursing Home Level of Care Determination as determined by the State of Michigan,
- Be able to live in the community safely with community and LifeCircles PACE support.
LifeCircles PACE must be able to show the State of Michigan and Center for Medicare and Medicaid Services (CMS) that a participant meets the level of care needed to enroll in our program. All PACE participants continue to meet the Nursing Home Level of Care in the future as well.
