Older adults often struggle to keep up prescription refills, or may loose track of when to take the right medications. We provide a pharmacist, nurse & home care coordinator to develop a system of medication delivery, set up and oversight customized to you.
Family caregivers might be overwhelmed in managing household tasks, medical appointments, nutritional needs and more for their loved one on top of managing their own personal needs. Our all-inclusive model focuses on coordinated, proactive care. The care team closely partners with the individual and their caregiver to provide peace of mind and simplifies the moving pieces of caring for a loved one with many needs.
Older adults might see several doctors, and it may seem like one doctor doesn’t know what the others are doing, leading to confusion or worse. Our team of geriatric experts meets daily to ensure all professionals are focused on what is important to you, even as your needs change. Since we authorize and coordinate all needed care, there is less opportunity for things to fall through the cracks.
Some seniors may have memory changes that lead to confusing behaviors, and stress for caregivers or spouses. Our entire team is ready to support caregivers in finding practical solutions to changing memory needs, and offering education so families know what to expect.
Living at home with out much support can lead to loneliness, isolation, unusual behavior, depression or anxiety. The team provides frequent contact and support from social workers, recreational therapists, behavior health, and more. Our day center provides an opportunity to connect with peers and develop a greater sense of purpose and meaning.
Physical therapy was coming into the home, but had to leave after a short time due to insurance time limits. After they left it has been difficult to keep up strength and mobility. Unlike other Medicare programs, our highly trained therapists can continue working with you for as long as you continue to benefit, and wish to work on your strength. Helping you maintain your strength and functioning over the long term is our goal.

LifeCircles is the participant’s sole service provider.  LifeCircles guarantees access to all Medicaid and Medicare services, but not to a specific provider.
