The LifeCircles Difference
Our primary focus is on preventative measures to maintain the health and well-being of our participants, allowing them to remain in their homes for as long as possible. At LifeCircles PACE®, we work as a team with our participants and family members acting as a central part of the team.
Along with the participant, the Interdisciplinary Team creates an individualized plan of care (called a ‘Life Plan’) to address specific needs. All Medicaid and Medicare Benefits are covered. Our innovative program includes, but is not limited to the following services:

Transportation Services
LifeCircles provides wheelchair accessible transportation to and from our centers, and to and from medical appointments as needed.
Health Clinic Services
Our onsite health clinic services include primary medical care, prescriptions, therapies, in-home visits, podiatry and advance care planning.
Day Center Services
Our Day Center offers recreational therapy, activities, exercise, socialization, spiritual care and daytime meals and snacks.
In-Home Services
Personal care, homemaking services, meal delivery, personal emergency response system, prescription delivery and care coordination
