September is National PACE Month, and at LifeCircles, we’re celebrating the ways PACE (Program of All-Inclusive Care for the Elderly) simplifies life for seniors and their families. PACE is a comprehensive healthcare and social services program that allows older adults to live independently in their communities while receiving the care they need.
If you’ve never heard of PACE before, you’re not alone. That’s why this month, we’re on a mission to educate our community about the benefits of this innovative program. PACE can be a game-changer for seniors who want to age in place while receiving comprehensive care.
We understand that navigating healthcare options can be overwhelming, especially when it comes to senior care. That’s why we’re here to help simplify the process.
LifeCircles: Unique End of Life Care
At LifeCircles PACE, we’re proud to offer something important: dedicated end-of-life care. This compassionate service provides comfort and support to participants and their families during a challenging time. We believe this care is essential to our mission of simplifying life for seniors at every stage.
End-of-life care at LifeCircles allows participants to remain in familiar surroundings and be surrounded by loved ones. It simplifies a complex process for families, allowing them to focus on quality time with their loved ones.
This unique service underscores our commitment to providing comprehensive care that truly addresses all aspects of our participants’ lives. By offering dedicated end-of-life care, we ensure that our participants and their families have support and guidance during one of life’s most challenging transitions.
How PACE Simplifies Life
PACE programs like LifeCircles coordinate and provide all needed preventive, primary, acute, and long-term care services for older adults within their own homes. This integrated approach offers numerous benefits that streamline care and enhance quality of life.
First and foremost, PACE participants benefit from having one dedicated team coordinating all their care. This means no more juggling multiple doctors’ appointments or trying to communicate between different healthcare providers. Your PACE team knows your entire health history and works together to ensure you receive comprehensive, personalized care.
Transportation is often a significant concern for seniors, but PACE eliminates this worry. We provide transportation not only to medical appointments but also to our PACE center. This service ensures that participants can easily access the care they need without relying on family members or struggling with public transportation.
At our two Day Centers located in Holland and Muskegon, participants enjoy a vibrant social atmosphere and nutritious meals. This aspect of the program addresses not just physical health but also the crucial social and nutritional needs of seniors. It’s a place to make friends, engage in activities, and enjoy delicious, health-conscious meals.
At-home Care
For those times when you need support at home, PACE has you covered. We provide in-home care and support as needed, allowing participants to maintain their independence while ensuring they have assistance when necessary.
Perhaps one of the most significant ways that PACE simplifies a participant’s life is by becoming their primary insurance provider. Since we are funded by Medicare and Medicaid, the vast majority of our participants receive LifeCircles services at no cost. This comprehensive coverage eliminates the stress of navigating complex healthcare systems and worrying about which services are covered. To learn more about who is eligible for LifeCircles PACE care, check out our criteria for enrollment or give us a call at 616-347-3477. PACE touches on all elements of care when it comes to helping participants live comfortably at home. We truly are a “one stop shop for all things aging!”
